Process of Registering a Company for PF
1) What is the process of registering a company for PF, which has an employee strength of 12 and has been established for 6 years?
2) I know that Form 1 is for the registration process, but what are Form 5 and Form 5A? What is their purpose?
3) Does the employer/HR need to go personally to the regional office to submit the documents?
4) Is there any kind of fee we have to pay for registration?

The process for registration is as follows:
1. Duly fill the application form "Form 1" as discussed, along with Form 5A, which is called the business application form. Along with that, you should submit a few supporting documents.
Required Documents for Pvt Ltd Company
If it is a Pvt Ltd company, then you should submit a copy of the AOA & MOA, a Board resolution, Company Address proof & copy of PAN, Directors' Address & ID proofs, the first invoice copy for proof of commencement date, an Attendance sheet copy for the last 2 months, a Salary statement for the last 2 months, a Shops & Commercial establishments registration copy of the factory license, copies of any other registration certificate, and a copy of the telephone bill, etc.
Fee and Submission Details
As part of the fee, there is no fee. However, they might ask you to pay around 2-3 months' contribution as an advance payment by DD, and you shall adjust those payments for further periods. The employer need not go; even HR can go, or you can arrange for a consultant to go.
Finally, the Inspector may demand some amount to issue the certificate.
