It is very necessary to conduct a perception survey of all employees regarding safety at regular intervals. The form is attached herewith. Please complete it so that you can have a better understanding of the Safety Culture, Safety Practices, Safety Improvements needed, etc.
I conducted the survey at the beginning of the Plant Start-up (8 months ago) and again last week. The results show the differences in the employees' understanding related to safety. It is important to conduct a safety survey for all employees, including contract employees, in the local language as well.
Please provide your comments. You can also add more points or suggest changes to any existing points.

Thank you for initiating such a thread and for sharing a good survey checklist. This type of survey was conducted at our plant by M/s. DuPont as an initial activity towards the Safety Excellence Journey. It was called the Safety Perception Survey.
You have covered almost all the points. You may consider adding the following points:
1. How often are structured training programs conducted in your organization?
2. Do you find these training programs valuable?
3. How would you rate the performance of your safety department?
4. Do you believe that accidents occur due to the ineffectiveness of the Safety Department?

Thank you for the very informative content. We had previously conducted an evaluation during the "Behaviour Based Safety" training, which covered some of these questions but not in full. After reviewing this information, I am planning to implement the same in our organization. I am confident that this type of survey may help identify many unsafe behaviors.
