Hi,
Based on my knowledge, the following questions may provide you with some ideas to prepare for the questionnaire:
- What is the reason for leaving your job?
- Can you provide an explanation?
- What could the company have done differently to encourage you to stay?
- If the answer is no, please elaborate.
- How do you compare your job with that of your co-workers?
- How would you describe your relationship with your immediate manager?
- How would you describe the importance of your job within this organization?
- To what extent does another company view your job as important?
- Were there any changes that influenced your decision to resign?
- What were some of the frustrations you encountered in carrying out your job responsibilities?
- Were there any factors that led to a change in your decision to resign?
- Did any training sessions help you?
I am currently employed in a company, and I believe these points are areas where HR professionals would focus to retain employees by addressing their concerns about leaving the company.

Hi Sunitha,
I have some more questions:
Do you face conflicts on a daily basis with the employee motivation techniques you are using? What motivation programs does your company use to retain employees? Do the team leaders, supervisors, or managers actively seek to motivate and retain employees? Are they trained for this purpose? If not, what type of training do they need to effectively motivate their team members? Additionally, what type of work culture helps the company retain its employees?
