One of my colleague had accident in working hours. How to intimate ESIC office & which forms use this case.please guide me

1. Sir, form of Accident Report can be downloaded from the website of ESIC or from the website of appropriate office of ESIC to which your unit is attached. The said Accident Report can also be submitted online as per procedure set up by ESIC.
2. I hope you will contact the appropriate branch office of ESIC to which the unit is attached in order to know online procedure etc.

Firstly come to know that accident is serious or not, if it is very serious and injured part of body which cause disablity of the worker, then within 24 hrs you would have to inform local office and submit online form of the esic site.

Dear Mr. Mayilraj,
In order to report an accident in ESI, you need to have the following details.
1. Date of Accident
2. Location of Accident
3. Start time & end time of shift
4. Exact place of accident
5. Cause & nature of accident
6. Nature & Extend of injury
7. Details of doctor who treated the patient
8. Witness (2 nos) & their contact details
Once you have all the above details, log in to ESIC site and create an accident report (form # 1) on the name of the IP. You can take the printout of the accident report and handover to your ESI local office. you need to do this within 24 hours of the occuring of accident in order to get the benefits for the employee.

Dear Colleagues,
Injuries caused to IP due to accidents during the course of employment is to be construed as " injury due to accident " is covered and he is entitled to every available ESI benefits. As he was in company's cab and met enroute with accident , its clear cut case.

Dear Mayil Raj,
Mr Harinandan has rightly advised you regarding the submission of the accident report. It is obligatory on the part of the employer to report all the accidents arising out of and in course of the employment within 48 hours of ordinary cases and immediately in death cases.The accident report to be prepared in triplicate ,one copy be sent to the local office and second copy to the Insurance medical Officer and third copy to be retained as office record. A copy of the Accident report is attached for your necessary action.
