HI Priti,
You won't get anything specific for a travel agency. Search citehr, I have a number of questionnaires posted under the heading of Employee Satisfaction Survey on page 3 or 4. You need to adapt them to your specific needs.

Are You Optimistic About Future Success with the Company?

If your survey indicates this is the case, you may need to consider adding more responsibility to your positions or offering the chance to earn additional income through incentives. This may help prevent employees from seeking positions with larger companies.

Do You Have Clear Understanding of Company's Future Direction?

As a small business owner, you may have a clear vision of where you want to take your company, such as wanting to open two new units within the next five years or becoming the leader in market share in the next two years. By sharing your vision with your employees, you can get them excited about working for the company, which can motivate them to increase their production.

Does Leadership Respond to Important Internal Issues?

You may be so busy with the day-to-day operations of your business that you may not notice if there are internal problems brewing beneath the surface.
