Dear Members Can anybody provide the prescribed form required to be filled in case a employee (having basic+DA more than Rs 6500) doesn\'t want to opt for the EPF deductions

You are required to fill out Form - XI under the PF Act, which states that the employee was not previously a member of the PF scheme. Please also provide backup documents such as their salary slip or an undertaking confirming that their salary is more than Rs. 6500.
