Dear all, I am working with a manufacturing company as an HR Executive. My query is, what is the use of Form No. 11 under the PF Act, 1952? My senior fills that form for most of the employees, and I don't know about that form.

The form no 11 is used for exemption under pf, means u wish to discontinue for contribution towards pf.

If any laborer is receiving Rs. 7000 as basic pay and is not interested in deducting his PF amount, we require a receipt declaration form. Please send PF exemption Form 11 as soon as possible.

Hello all,

I filled out Form 11 without knowing much about it at the time. Now, I would like to cancel it to utilize the PF facility. How can I go about canceling my request? And is it beneficial for me to do so?

My salary is 34,000 rupees per month.

Yes Form No 11 is for an employee whose basic is more than 6500/- that is to show to the authorities that he is exempted trom paying his contribution.

Dear Mr. [Name],

The Form 11 is a declaration by the member for his membership of PF. If you view the Form 11, you will understand the necessity of this form. The member declares if he has withdrawn his earlier PF, his previous PF number EPF and EPS, or he was not a member of EPF.

If he has his first employment and PF wages above Rs 15,000, he will not be a member of the EPS scheme. Hence, the declaration is important.

In Form No. 11 under PF Act. 1952 mention Scheme Certificate. What is the purpose we are using Scheme Certificate. And also attach the Specimen of Scheme Certificate.

Dear All,

Scheme Certificate:

If a member is not working in any establishment where PF is applicable, they may obtain a Scheme Certificate for EPS. The Scheme Certificate includes details of your service before November 1995 and after November 1995. It also contains other family details, the average salary for the last 12 months, and the salary as of November 1995.

With the certificate, you can claim a pension upon reaching the age of 50/58, subject to fulfilling other conditions, or you can choose to withdraw the EPS amount.

I am working in a construction company as a daily laborer, and my basic wage is $11540. They deduct 12% of the basic wage for PF. Now, they are asking me to fill out Form 11. What is the purpose of Form 11? After submitting that form, what will happen? Can I claim my PF amount? Kindly advise me on whether I should fill it out or not.

Dear Mr. Dhinesh,

The Form 11 is a declaration given by the member when joining the establishment. In the declaration, details of your previous establishment are provided, including the code number and PF number, among other information. If the company wishes to update their records, which they have not done previously, they should fill out Form 11 and submit it to the company.

Form 11 is not an exemption form, but a declaration from the employee, where the member declares that he or she has an active EPF account from past service(s) or not. If they have any account in the past, they will declare it on Form 11 and continue to contribute to EPF in present employment. In case their basic salary + DA exceeds 15000/- and they have not contributed at any time to any provident fund, that will appear in the Form 11 declaration.

Employers have to offer the EPF benefit to all employees. However, after submitting Form 11, if the member wants to remain non-contributory, then the member has to submit a letter to the employer, stating the willingness to remain non-covered due to any of the following reasons:
a) basic salary + DA exceeds 15000
b) member has no past EPF contribution record
c) member has crossed the age of 58.
