I am a Newly joined as a HR Executive and don’t have any experience of HR job. So Plz update me about Matenity Leaves or if any form required then from where we will get that form.
What is the difference between Form A & B for Maternity Process. Our office is in Gurgaon, Haryana so for Haryana which form should be required to download.
Hi,
1. Form A for muster roll .
2. Form B Medical certificate stating her pregnancy from authorized medical practitioner.
3. Form C Death certificate ( During the pregnancy)
4. Form D In case delivery miscarriage.
5. Form F Receipt of Maternity benefit.
6. Form E Notice under section 6 of the Maternity Benefit Act for proceeding on Maternity leave.

Dear Asha For taking maternity leaves, application supported by medical certificate should be sufficient.
