Hi,
Firstly, no two company policy can be same, prepare a draft and I can help you edit it. I can answer your basic questions but a policy is a lot more than the points you have mentioned above.
Ideally IT firms dont work on Saturdays. If they happen to a comp. off (extra paid day) is added to their leaves.
Work from home you can allow 2 in a month or so.
For Salary structure you have components like : Basic, HRA, Conveyance and Medical.
Also in deductions you will have EPF, ESIC and PT
Break timings can be half hour lunch and two tea breaks of 15 mins each.
Leaves can be 14 festive leaves and 21 Leaves (sick, casual)

As rightly pointed out by Foram above, you should search inwardly about what employees expect from Management. Conduct a survey amongst employees but inform them that all their suggestions will not be implemented but will be considered. Keep yourself updated by reading HR related articles in internet... That is the best way of implementing new initiatives. No copy paste of policies. By the way, many people still mention Days of Leave as Leaves. Leaves can be found only on trees and not in offices. There is no plural for Leave... it is always Number of days of Leave, Types of Leave etc.
Best wishes
