Planning to quit my job after 2 months due to mental health issues. How should I plan my exit?

I am 22 working in a marketing agency. I've joined this new agency 2 months ago and since then my mental health is suffering.

I've discussed with management on workload and projects 2-3 times.

I've explained my challenges to my manager and he gave me tips to work on. It worked for 2-3 days, however, I am on the same thought pattern again.

It's been 60 days since I am suffering from the workload, not able to deliver tasks on time, missing deadlines, doing mistakes, overthinking my decision of joining, negative self-talk, losing confidence, losing interest in work, and many more. Sometimes, I think I should take a career break, get refreshed, and explore new things in my 20s. I don't have energy to give interviews while working here full time.

I am trying spiritual way, mediation, god prayers, exercise for improving the situation.

I also took the medical way. My doctor prescribed me some medicines for 15 days and asked me to take 1-week rest.

I've applied for 1 week's leave with a doctor's prescription however, HR is asking for a medical certificate in order to approve the leave request. In our country, private doctors can't give medical certificates, only government doctors are allowed to do so.

I've been planning to quit my job for the last 2 weeks. I feel the guilt of taking salary and not being able to perform tasks, I don't feel good when my co-workers doing my work due to my absence. What reason should I mention in the resignation email? How should I plan my exit? I am so confused to think about it.

Dear Rohan,
You discuss with your HOD, that you want to quit job owing to health issue. You inform that the doctor has advised for complete medication with physical rest for 2-3 months(have the prescription). There is no need to get emotional better devout your time for quick recovery.
