Draft for invitation of domestic enquiry for absenteeism

Can anyone help me with the draft for invitation of domestic enquiry for absenteeism

Pl. specify whether the charge sheet issued or not? Also, the applicable rules have to be considered before making any suggestion

Dear Sir,
Its called Notice of Inquiry which should contain the following:
a) The need for inquiry as not satisfied with the explanation in answer to Show-cause letter
b) Date, Time and place of inquiry
c) The Charge or charges, which should be specific
d) Request the complainant to bring witnesses, if any.
e) A sentence to say if the complainant fail to attend, the inquiry will proceed Ex parte
