New Registers Under CL(R&A) Act 1970

Sir,
Can any body let me know about the new format of registers / records to be maintained under Contract Labour (Regulation & Aboilition) Act, 1970.
Kindly provide registers.

Pls let me know the following queries:-
1) Min. no. of labours for contractor registration?
2) If workers of the contractor have worked more than two company. Than, is it necessary to show min. labours each for registration/licence.
3) Is it necessity registration/licence from Different ALC?(If the companies are under different ALC)
