Devika the questionnaire i found is ok-ok. i would suggest to make it more detailed and exhaustive and include the points like what results they expect while using the test, how much accruacy orgn think is coming by using the said test, are the results decalred to the participants, are the test used in assessment center also, etc etc.
see if you can accomodate the above points nicely.

Dear Devika,
Not to disappoint you, but
Forming of QUESTIONNAIRE itself is a phenomenon.
Like few basic things are
1. A questionnaire has to have a COVERING REQUEST ( Part of Questionnaire )
2. Methodology to be followed while framing like you may opt FUNNEL METHOD.
3.A questionnaire should contain in between 15 to 25 questions.
