Devika, the questionnaire I found is okay. I would suggest making it more detailed and exhaustive. Please include points such as the expected results when using the test, the level of accuracy the organization expects from the test, whether the results are disclosed to the participants, and whether the test is used in assessment centers. See if you can accommodate the above points nicely. Response awaited.

Dear Devika,

Not to disappoint you, but forming a questionnaire itself is a phenomenon. There are a few basic things to consider:

1. A questionnaire must have a covering request (part of the questionnaire).
2. Methodology should be followed while framing it; for example, you may opt for the funnel method.
3. A questionnaire should contain between 15 to 25 questions.
