We are planning to design a mobile, laptop, internet connection, and telephone undertaking form for employees. Please share the format if anybody has it.

You can have this form, no doubt, but it is a small part of the overall policy on "Allotment of Company Assets to Employees." You need to have this policy well in place. The format of the "Asset Allotment Form," or in simple words, "loan card," is an appendix of this policy.

Mere having this policy is not sufficient. One more thing you need to do is the proper maintenance of the Fixed Asset Register (FAR). If the FAR is well maintained, then the audit of the fixed assets becomes much easier.
