Organizational Survey - Doc Download

M a student of MHRM placed as an intern in ESI hospital vadodara. I have to do an organizational survey in the hospital consisting of 40 doctors as my sample size. please help me design the framework of the survey. and if possible provide me with sample questionnaires.

Dear Jayu
I have attached your document with some general comments. I am sure others will comment on the content.
There is some confusion: Are you administering the survey to all the staff (as per the introduction to the survey) or just the doctors? If it is the latter, are there only 40 doctors in the hospital or more? If there are more, how will you be selecting the sample?

thanks for the modifications, but the 5 point scale was kept deliberately because its a bureaucratic setup, they should not anytime feel we are cornering them. and we just had Doctors as our sample and yes there are only 40 doctors including hospital and dispensaries.
we didn't include other staff members because of their lack of knowledge and understanding towards such serious issues as they are less educated.
