Dear Nabeel,
You have not said who you are (a student, a researcher, etc). You also have not given any introduction (as to why it is being undertaken and how it will benefit the responders)to the survey.
If I may take this opportunity to inform other researchers, the questionnaire should have been piloted before publishing here for responses. For example, asking for personal questions at the outset puts respondents off. I normally will not divulge my age, etc for surveys which are not well designed and give details of who is collecting and for what purpose (though you have put a note about the way it will be treated confidentially).
Also, requesting people to use a different coloured font will put people off. Instead of the narrative type, had you used a table for the Likert type questions it would have improved the layout and made it easy to respond by ticking the boxes.
These are my quick observations.
