Dear Mr. Theniniyan,

You have joined our organization on 01/12/2004 as an Apprentice Trainee (Line Inspection) in our Quality assurance department and working in our Unit II.

As a Line Inspector your main responsibility is to inspect the machined components in PMD division as per the control plan and inspection plan and record them in the relevant Inspection records.

On 01/09/05 you have inspected CPU Frame component first off samples visually comparing it with master sample without checking the dimensions of the component as per the control plan and inspection plan and gave clearance for production. This lead to the production of 2000 defective components which has resulted in financial losses to the company. You have also failed to maintain the relevant inspection records for this component.

Your above act shows that you have failed in carrying out your job requirements as instructed by your department heads. Your above act of indiscipline is a serious misconduct, which warrants severe actions against you.

Since you have accepted your mistake and assured that you won’t repeat these kind of mistakes in the future, we have decided to let you off with a warning.

Hence you are hereby warned not to repeat this kind of misconduct in future, you are also advised to strictly follow the work instructions. In case if you are repeating these mistakes again, we will take severe actions against you as deemed fit.

Hi saravanan,
i think if u handle the things softly so no need to issue the warning letter just call the person and tell him verbally not to repeat such mistake. issuing a warning letter is itself a harsh action it can not be soft at all & it is given when the issue is really serious or after being reprimanded the employee repeats it.
