Instead of asking for a completed project, you should conduct a literature search, form your aim and objectives and then do the study yourself.

You may have a clear idea but we are not privy to that. Hence, kindly give us as much info about your project on T & D as possible. I am sure HR experts will agree with me that it is a very broad area and the questionnaire will also depend upon the type of organisation, its activities, and the level of staff in mind, etc.
