I have few doubts:
* what is the establishment account number, mentioned in Form A?
* Is it enough if we take the print out of this form and send it?
or is it mandatory to get the original form from Labour Welfare Board office?
* Also let me know whether we will receive any receipt if we send it by post.

*while sending the contributions first time, you leave the column as blank.
and when you are receiving a receipt for the payment you can find account number in that - which is meant for your establishment's account number and the same shall be mentioned in the coloumn in the forthcoming years.
**Yes.you can send the form after filling in the xl sheet itself and take print out. Not necessarily you should run after anybody for forms.
***On receiving the cheque, the TNLabour welfare Board will send the receipt by post - but it will take two three months time.
