To download the C-11 certificate form from the ESIC portal, please follow the steps below:
1. Visit the ESIC homepage by typing the ESIC URL in your browser. The official ESIC website is https://www.esic.nic.in.
2. Once on the ESIC homepage, look for the 'Services' tab on the top menu and hover over it.
3. Under 'Services', find and click on the 'Insured Person' option.
4. You will be redirected to a new page. Here, look for the 'Download Forms' option and click on it.
5. You will now see a list of downloadable forms. Scroll through this list until you find the 'C-11' form.
6. Click on the 'Download' button next to the 'C-11' form. The form will be downloaded to your device in a .pdf format.
7. Open the downloaded .pdf file and print it out.
Please note that you must have a PDF reader installed on your device to open the downloaded form. If you don't have a PDF reader, you can download one for free from the internet. Also, ensure that your device is connected to a printer before you attempt to print the form.
