Hazard Severity Likelihood PPE required
Chemical Spill High Likely Gloves, Goggles, Respirator
Physical Injury (falling objects) High Likely Hard Hat, Safety Boots
Exposure to Extreme Heat Medium Likely Heat-resistant clothing, Cooling vest
Exposure to Noise Medium Likely Ear protection (earplugs or earmuffs)

Steps to Create a PPE Matrix:
Identify Hazards: First list all of the potential hazards in your workplace or surroundings
(e.g., electrical shock, falling objects, chemical exposure).

Assess the Severity of Each Hazard: Determine how severe the consequence would be if the hazard occurs
(e.g., minor injury, major injury, death).

Assess the Likelihood of Each Hazard: Estimate the likelihood of each hazard occurring
