Received a show-cause notice for late registration of an employee in ESI. But I don't know about which date is mentioned at the time of registration as date of joining. How to reply to the department? and what are the required documents?

For ESI Registration one require proper details like employee name, Employee DOJ, nominees name along with relationship and DOB, present and permanent address of the employee, bank details of the employees. These are the prerequisite data one required for ESI registration. So without knowing DOJ of employee how you will be able to register?
You can reply stating that you were not able register on time due to lack details. You can prepare ESI form template and insist for employee to fill the form with signature so that it would be for you to process ESI registration.
