Dear Members,
We have been processing two death claims from EPFO. The cases has been going up and down between us and EPFO (much like the popular SITCOM of early 2000's - "Office Office").

This time the claims have been returned with few observations, some of which are listed below:-
a) Provide F/10 and F/5.
I could find F/5 (Return of Employees Qualifying for membership of EPF, EPS & EDLI for the first time during the month of ....). My doubt is with present day online systems is this return still required to be rendered? If yes, then on what occasions? Is this to be rendered every time we enroll an employee in PF?

b) Provide F/10:
I could not find F/10, I got only F/10D. Is this same as "Descriptive Roll"? Is F/10 & F/10D different?

c) Provide past service if any or provide non-employment certificate.
How do I provide past service record?

d) Provide Descriptive roll of claimant & dependent.
Is this same as F/10 asked for in para (b) above. Or is this F/10D?

e) No affidavit. Birth certificate of claimant is mandatory.
The claimant has provided birth certificate in something that look like an affidavit on a Rs. 100 Stamp paper (Year of birth is 2019). (Screen shot of certificate issued by local authority and relevant sections / rules quoted in certificate are attached). To me this looks fine enough and cannot understand insistence on a "birth certificate". Any inputs / suggestions on this?

Have you considered going to the PF office or taking the help of a PF Consultant since you are obviously not acquinted with the requirements?

a) Provide F/10 and F/5.
If they asked for the form, you have to give it, whether it already in the system or not. Yes, it is filled every time an employee joins, but it happens online. I think there must be an option to down load the same on the Portal.

c) Provide past service if any or provide non-employment certificate.
How do I provide past service record?

I think they are asking for details of the period when he was not working. Was he sick and not getting paid before death, for example? The family will probably have the details of his previous employment or employment gaps.

No, the birth certificate is important. The claimant needs to get it. A certified copy of the screen shot will generally not be accepted. It can be faked very easily.
