I need a format for the Top executives for submitting their report after orientation program. Generally once the Top executives join, they are sent for an orientation program across the country to know the Operations process. Once they come back, they need to prepare a region wise report. A professional method of format to submit these reports would be of great help.

You could have given a little more information about the nature of your industry, your company's finished product or service, the definition of "top executive" and so on.
Over and above, we the members of this forum are outsiders and we do not know the internal dynamics of your company. However, if you upload an outline of the orientation programme for the top executives and prepare the draft of the format of the report, then it will become easy for the other members to suggest you the corrections.
