What documents do I need to request before leaving a remote company?

Background:

I am from India and have been working full time remotely for a Netherlands based company for 5+ years, It is a small company (5-10 employees). lets assume the company is called 'company-x'.

Initially I had joined as a 'Technical support representative' for 'company-x', then after 1 year the company started a SaaS product and I was switched to 'Developer' role, further after another 1 year I was promoted to 'Full stack developer'.

Also on every title change or salary increase, I was asked to sign a new contract which included the salary amount (In US dollars) and role.

Initially for couple of years the salary payments were made via paypal then later they switched to wise.com (formerly transferwise). Also I did'nt get any salary slips upto date (I never asked for them).

Whenever Wise (Transferwise) sends a payment to me, I recieve an email in the below format, But in bank statement it does not show the company name or reference (payroll).

I am planning for a job switch, so I have a few questions for the same.

My Questions:
1. What documents do I need to take when I leave the company? (all the documentation is managed by the CEO, there is no dedicated HR dept, so I need to know what I should ask for)
2. Based on some research , It seems company name must be there in the bank statement, but in my case It does not reflect, would it affect in any form when I approch new employer? (based in india or foreign?)
3. I dont recieve form 16, My CA paid the taxes by referencing me as self employed. Similar to 2nd Question, would this be a problem?
4. Is there a template for salary slips specificly meant for remote employees? if so please provide me the same so I can pass it over to my CEO in future if required.
5. In experience letter/certificate all titiles/positions thoughout my tenure need to be mentioned? If there is a template for this, it would be really helpful aswell.
