Its called Notice of Inquiry which should contain the following:
a) The need for inquiry as not satisfied with the explanation in answer to Show-cause letter
b) Date, Time and place of inquiry
c) The Charge or charges, which should be specific
d) Request the complainant to bring witnesses, if any.
e) A sentence to say if the complainant fail to attend, the inquiry will proceed Ex parte
