Notice period not served and relieving formalities not completed - how to handle HR?

The question seems to be very cryptic. What is the length of the notice period and whether there is any buy out option? Why notice period was not served? whether the employee submitted the resignation with immediate effect with the option for buy out of the notice period? Whether the employee stopped coming to work as soon as he submitted the resignation? Whether his resignation was accepted subject to the fulfilment of the notice condition by the employee or simply kept without any action?
