How PF is calculated in IT companies? Calculating as 12% (Basic+medical+conveyance+special all). Is it correct?

My salary structure is basic+HRA+medical+conveyance+special allowance. Then for calculating PF, They are calculating as 12% (Basic+medical+conveyance+special all). Is it correct? I heard that medical and conveyance are not to be included? Is it correct? Why DA is not calculated?

On DA PF is payable. But in your Pay factors DA is not there
