if the private hospital is referred by ESIC. then they will settle the bills directly, no communication needed by employee
if the employee or employer did not intimated ESIC regarding the treatment in private, then it is tough to get claim settled, though the employee may visit the AMO (ASIC) of their area, they will provide the requisite forms to make claim.
