Dear Team,
I want to take the acknowledgement from one of my employee who is suffering from fits. initially we thought to remove him from work (as he is working in maintenance department) but he requested a lot so we thought to take the acknowledgement from him and his family saying that if something happen to him in future while working on moving machine, electricity and machine parts, its their responsibility.
please help me with draft how the acknowledgement should be...
