In the format of specimen signature, the website mentions 'Please upload for all employers and for Authorised Signatory if any' and place is given for all 4 employers. The establishment in question has only one employer - so why they have given 4 employers to fill.

unless it is a proprietorship firm, there should be more than one person in establishment , viz partners, directors, trustee etc
all concerned persons data is needed in form 5A, at the top of this paragraph it ask for no of employers, if there is filled 1 it will display only single if it is filled 6 then 6 boxes will get displayed for submission
