In the format of specimen signature, the website mentions 'Please upload for all employers and for Authorised Signatory if any' and place is given for all 4 employers. The establishment in question has only one employer - so why they have given 4 employers to fill.

Unless it is a proprietorship firm, there should be more than one person in the establishment, such as partners, directors, trustees, etc. All data of the concerned persons are needed in Form 5A. At the top of this paragraph, it asks for the number of employers. If "1" is filled, it will display only one; if "6" is filled, then 6 boxes will be displayed for submission.
