Not clear However please refer the modified one as per your draft. In case if you have certified Model Standing Order or follow model standing order you can refer that clause here( serious misconduct as per our Standing.........). Advise the employee to sign the second copy of the letter as token of acceptance. In case of any previous warning you can refer the same here.

However it has been brought to our notice that you have not reported to office on ____ and had extended your leave without any prior information to your immediate Superior /HOD. It is very unfortunate that a person of your stature who is supposed to be an example for your subordinates, extended your leave without any prior information. This had caused unnecessary hardship to your Team.

You are hereby directed to refrain from proceeding on leave /extending you leave unauthorizely henceforth.
