It's simple, just list out those items require maintenance and put them in a direct mail to the concerned in-charge of Co.accommodation maintenance. While doing so it's important to state/briefly describe the present state of those which require such maintenance. May be you have to ask the official to inspect and see for himself what is the condition of those facilities and let them make a call. If necessary print your mail, sign it and courier to the concerned official for their record & follow up action.
