Hi All, I am working with an IT Company. We do have Variable Performance Bonus in our salary structure. As per the process Team lead has to decide whether bonus will be give or not. I need one form, which Team Lead can share with HR with the recommendation that whether we should give the bonus or not with reasons.

Is't linked to performance based on individual's performance or entire team together to be shared among the members of the team.
Please indicate the activities, threshold limits, formula for computation of VPB.
