ESI benefits after resignation - Please suggest if eligible, what process I need to follow to get ESI benefits?

Actually I am relieved from company in December 2018. We have ESI dedication in my company. Now I want to use my ESI. I never take any ESI number or ESI card from company. Now am eligible for taking ESI or not? Please suggest if eligible, what process I need to follow to get ESI benefits.
