Sir, I am an ESI insured employee. on 13/12/2018 my wife delivered a boy with operation in private hospital. on 29-12-2018 in my local municipality given a birth certificate with the names of parents based on aadhaar and mentioned delivered a male boy to me. We are planning to put a name in third month of his birth, because of my misses health is unfit. In the claiming form also just asked a birth certificate. There is not mentioned birth certificate with the boy name or with out the boy name in the claiming form. Sir, Now i can claim the confinement charges Rs.5000/- under rule 56-A of ESI Scheme. Please give your valuable suggestions. I am very thankful to you sir.
