KYC formalities of establishment would be resubmitted , ask your CA to make compliance, as HR does not has access to many documents, viz how the proprietary firm got abolished / closed or merged under llp is known to CA only
if proprietary firm got closed then it is better to obtain a fresh ESI/EPF regn and surrender the older by producing the closure of business documents
