This is a very generic question.
Causes could be:
Frustration at events
Perceived feeling of being treated unjustly
Feeling unrecognised or taken for granted.
Ill treatment in office.
Probably you need to call the employee and have a heart to heart talk, shedding all barriers of rank/status/designation and try to read his mind and what is said and unsaid.

Dear Anony,
Following could some of the reasons according to me:
1. Differences with management or with reporting authority, could be in working style or approach towards business/conflict of ideas
2. Employee could be in burn out stage - Lacks motivations or focus
3. Stress due to office or personal issues
4. Lack of belief in the management
5. In retaliation to organization's inhuman practices or rude behavior of the management/reporting manager

Yes right query.sir
A. Responsible. Employee in the beginning. Later forgot all responsibilities.
No discipline control mechanism
Though. The norms are like paper tigers.
No simple medium ultimate punishment s laid on any misbehaviour or mistakes.of any employee.
Who cares if there is no warnings whenever did mistakes. Like a class room teacher and students relation should keep always management and employee s.
