Prepare a letter to PF commissioner for mistake while bulk uploading the details

I am an employer, while bulk uploading the details of more than 100 employees in pf site for generating UAN number, their date of birth and date of joining are mistaken. Plse help me to prepare a letter to PF commissioner regarding to this subject. Thank you.
