Kindly help me in writing a tabular for an employee who provided fake certificate and got promotion. I have to call comments of the employee.please provide some formats.

Can you please provide little more information as to what exactly is the issue? Who was recruited, when he/she was recruited, when it was detected that the certificates were fake, who detected it, how come it escaped from the scrutiny of the recruitment department and so on.
What you mean by "tabular"? Dictionary meaning of this word is "of or relating to the table".
Lastly, about maintaining anonymity. Your post does not merit making your post anonymous. Just because the facility is available, it need not be used. If you are seeking advice from seniors then you need to be open and transparent also. Neither your post is that professional.
