As per your query you want the nominations from the team on their leader, but what is your/company's role in identifying the leader?
Ideally identification of leader should be on the basis of actual skills which vary differently to individuals.
I don't think filling up questionnaire would serve the real purpose unless you assess actual qualities by observation.

Leaders are not chosen through questionnaire but on the basis proven/potential ability to lead a team. He should have superior knowledge of the activity, each team member's strength s and weaknesses and ability to lead by self-example with focus on results to be achieved .He displays initeative, natural liking for people, lovable attitude and acceptance by team members.
Find out who such person is by day to day observation and give the responsibility for results.
