Hello Everyone! I want to know about the forms of maternity act, how it fill and when to submit it. it is new for me. I go through the act but did not understand much.

there is no format as such, but employee needs to intimate to the employer along with her medical reports for claiming the benefits.

Sir, There is a From-A (See Rule - 3) Muster roll. and the other ones like Form-L,M,N,O.
My Question is that as an employer we have to Maintain Form A monthly basis ? or if any Woman Give notice under section -6 , than only we have to maintain ?
and the remaining form L,M,N,O , we have to maintain yearly ?
