Please share me your inputs for, Can an ESI dispensary refuses for treatment for an eligible employee if employer defaults in ESI both contributions? Any notification references available?

Yes, ESI dispensary can refuse for treatment in case no contribution is made for the specified period. It is clearly specified under the act as if the payment contribution is made to ESIC as per mentioned scheduled in the act only then the employee can take the benefit. For more details you can refer ESIC act.
