Hi i have lost our company user ID and password. when i submit the letter for New user Id and password. Department asked me to submit Form 9A. I know that in Bangalore the department issue form 9A book. My question is, if there is any format need to be filled for collecting the form 9A with covering letter or only covering letter with company seal and signature is fine. Also please share the covering letter format.

Dear Anamika, what is this Form 9A? Which department is asking for the same? you say you have lost company user id & Password belonging to which portal.There is option "Forgot Password" you can send mail from your official mail id.
