Hi Harish,
Following documents are considered for KYC –
• National Population Register
• AADHAR
• Permanent Account Number
• Bank Account Number
• Passport
• Driving License
• Election Card
• Ration Card,
So, Bank KYC means your Bank details -Bank Name , Account Number,Etc

Dear Harish,
Go to your banker and ask for KYC form, they'll give you the KYC format in their own stationery which can be used. You have to submit it together with your Id proof such as PAN, Driving licence, voter ID, passport and address proof like Aadhaar card. You have to present them originals, xerox copies may be refused. Now a days all are asking for Aadhaar card. So if you don't have your Aadhaar card immediately obtain. Even new SIM cards are only issued linking with Aadhaar No. and thumb scanning. If not submitted activation will not happen.

if you have PF UAN, You have to furnish your KYC-SB A/C #, IFSC Code #, Aadhaar # to your Employer, who in turn has to upload the same & approve. Please log in to member Portal & check what KYC Documents have been filed.

I forgot to tell you Harish, By chance if you don’t know the full form of KYC = KNOW YOUR CUSTOMER.
