As per EPFO Notification No. Manual/Amendment/2011/Pt./163, The Central Provident Fund Commissioner prescribes Composite Claim Form in Death cases by replacing existing Forms No. 20, 5-IF and 10-D. This new Composite Claim Form may be used by the claimant in case of death of a member, to apply for claim of provident fund, insurance fund and monthly pension in this single form.
