Fill form 5A with reqiusite attachments, and submit the hard copy to department,
after filing form 5A, check that your LIN is approved or not, f not then very, add verifier and submit for verification.
if LIN is approved then login in PMRPY and submit the lin related details and bank details. and verify the same digitally
now try with EPFO, if everything has been in order, EPFO will work smooth with your DSC
