What is the present formula of EPF pension scheme?

Hello all,
My EPS contribution Rs. 15400 shows in passbook. but i withdraw only Rs. 9000. Now its shows you already claim eps by submitting 10C online. Please help to get all eps amount. Last two organisations eps contributions are 9400 & 6049 respectively.
