I m currently working in Gujarat circle company , i have one quarry about PF withdrawal , details are below,
Employee working last 30 years old in our Organisation , he is die (20.09.16) for out of company premises , for natural death so please suggest which form fill up for PF Withdrawal (Form 20) and Pension (10D) and procedure for all related to form ie gratuity, and which document given by EPFO and ESIC related to pension and PF amount withdrawal.
Please suggest me which form and which doc attached in all form.

2. Nominee ID Proof those are nominate in p.f form -2 revised.
3. Employees ID Proof.
4. Police FIR copy.
5. Muster Roll copy for employees .
6. ALL DOCUMENTS ARE ATTESTED BY COMPANY AUTHORIZED PERSON.

Please submit following documents under EPF. 1) form 10-D (widow/children pension),2) form 20 for Provident fund wih drawal and 3)form 5 (IF) for EDLI along with original death certificate, 3 photos,Bank pass Book Xerox copy, family servival certificate. If diseaed employee have minior children's submit separate bank account for pension, birth certificate's to be attached with above claim application.
