Your post needs further clarification. Do you want this letter for you or you wanted to issue to someone from your company?
What is the position of this person? In which department does he work? Will there be change of department? Will there be change of designation?
Have you prepared Job Description (JD) of the new position? If yes, then you may issue the letter stating that from which date the change will take place, whom this person will report to. Attach the copy of JD to this letter.
